When contacting us we will require the following information:
- Name of member,
- Date of death,
- National Insurance number,
- Next of kin name, address and contact number.
Verification of the appropriate certification and paperwork is required to assess eligibility of any death benefits. The Clwyd Pension Fund is responsible for the payment of survivors pension benefits and any Death Grant Lump Sum.
